We offer same day appointments at all of our locations for urgent care needs. Our priority is to make every attempt possible in offering you a same-day appointment with your primary care provider at your home clinic. Please keep in mind that a same-day appointment may be offered to you at one of our other clinics with another provider to assist you with your urgent healthcare needs. Providers at each of our locations have access to your chart, and are happy to help you.

- Please call ahead to schedule an appointment for routine care. (Appointments can be scheduled up to 3 months in advance.)
- Save time and fill out your New Patient Registration Forms ahead of your appointment, Click HERE.
For your first appointment, please bring the following items:

- Your insurance card (if applicable).

- A list of your medications including over-the-counter medicine, vitamins, or herbal supplements. (Click here for an “Ask the Doctor” pre-visit checklist!)

- Immunization records (for your child’s visit).

- If applying for our sliding fee scale, please bring your current pay stub or tax information for income verification.

MyChart LoginWhat is a No-Show? A “no-show” occurs when you do not attend your scheduled appointment and do not contact us at least 24 hours in advance to cancel or reschedule.
Why Does This Matter? When you miss an appointment without notifying us, it prevents us from offering that time to another patient. Your consideration helps us provide timely care to everyone.
Our No-Show Policy:
- Established Patients (anyone we have seen once or more): if you miss three (3) appointments within twelve (12) consecutive months without 24-hours’ notice, we can no longer provide you with care at any of our Full Circle Health facilities.
- New Patients (patients we have never seen): if you cancel and/or miss two (2) appointments within the first twelve (12) consecutive months of being a patient without 24-hours’ notice, we can no longer provide you with care at any of our Full Circle Health facilities.
How to Avoid Being a No-Show:
- Call us at least 24 hours in advance of your appointment time if you need to cancel or reschedule.
Our team is happy to assist you in finding a time that works better for your schedule.

As part of your online check-in process in MyChart, you’ll be asked to complete a brief Patient Voice Questionnaire (PVQ). This simple step gives you the opportunity to share your top concerns, goals, or questions before your visit begins — so your care team can tailor your visit to meet your needs.

- Highlight any symptoms or concerns
- Set a goal for your visit
